We are an independent service improvement consultancy.  Every day across our society there are thousands of beautiful, successful interactions between professionals and the public, their patients, clients, pupils, and their families.  There are also times when near misses occur, when humans rescue a situation by recognising a risk.  Most of the time an adaption is made.  Sometimes though the systems which professionals work within make it difficult and unanticipated harm occurs.
