We are an independent service improvement consultancy. Every day across our society there are thousands of beautiful, successful interactions between professionals and the public, their patients, clients, pupils, and their families. There are also times when near misses occur, when humans rescue a situation by recognising a risk. Most of the time an adaption is made. Sometimes though the systems which professionals work within make it difficult and unanticipated harm occurs. That is where we can help. At Systems Safety we have the experience, skills, and tools to make sense of incidents and work with all of those people involved to provide the much-needed answers.
Dawn is a sociologist. Much of her work has been in academia as a researcher and lecturer within healthcare, where she developed the concept of ‘high reliability parenting’ after undertaking research with families of birth injured infants. Dawn has worked at a number of universities in education, social and medical sciences. Her most recent work was with the Patient Safety Academy in Oxford lecturing and researching healthcare investigation and Human Factors. She currently supports the Clinical Human Factors Group (CHFG) as the Programme Manager. Dawn was seconded at the HSIB to establish the education function where she developed the concept of Investigation Science.

We examine the critical and risk factors which contribute to harm. We work with our clients and their service users and stakeholders to identify their needs and actions which will improve the safety, efficiency and quality of their work.
We work with public, private and third sector organisations to undertake investigations, research, and audits to identify the factors which threaten safety, efficiency, and desired outcomes. We provide management advice by consultancy and training.
